Church Use Request Form
The people of Avondale United Methodist Church are very aware of our place in the community. Established almost 125 years ago, Avondale has stood on the corner of 40th Street and 5th Avenue South (originally called Avenue E) and served the people for generations. We hope to continue the tradition of openness for at least 125 more years. Our facility has served as home to many organizations and has provided 'a place to be' for many more during its long history. Boy Scouts, Girl Scouts, "After School Care" programs, various athletic teams, classes, organizations, and groups have met here and felt the welcoming acceptance of our church family. Many programs, recitals, celebrations, and milestone events have been celebrated under our roof. Other groups have found a safe haven in which to store their equipment and supplies. If a civic or service organization that you represent has a facility need, you are invited to check with us concerning the availablilty of our facility for the use of your group. First: Contact the church office (592-3739 or avonumc@aol.com) to determine that there is no conflict with already scheduled events for the date, time, and specific area of the church you need. Church members scheduling "new" church events or meetings should also check with the church office to avoid conflicts. Second: Contact the chairperson of the Trustees, Tim Smith, to determine if your organization or event meets the church's criterion. Third: Wait for authorization from the church office that your event has been scheduled. To facilitate the process, you may choose the complete the following form and submit it by email to the church office. If there is no conflict of time and space, the secretary will forward your request to the Trustee chairperson for consideration. You will be informed by email as soon as a decision is made.
